Dear Parent/Guardian,

The Wallkill Central School District registers all students at the District Office located at 19 Main Street , Wallkill , New York 12589 .  To register your child or children, please bring the following items with you, as we cannot process your registration without these documents.  Please be advised that your building principal will be contacting you about student placement, since class sizes may reach maximum numbers and your child may have to attend another elementary school outside your attendance area.

Student Information

•    Birth Certificate [Original]

•     Social Security Card

•     Immunization Record

•   Report Card from Previous School [include name, address, and telephone number of school]

•   Custodial Papers (if appropriate)

•    IEP (if appropriate)

Proof of Residence in District

                Normal Circumstances [Buying or Renting]

                        Two (2) Proofs of Residency [must have street address, no PO Box]

•    Mortgage

•    Rental/Lease Agreement

•    Utility Bill [ Central Hudson , Telephone, Cable]

•     Driver’s License

•     Pay Stub with new address

•         Voter Registration Card

                        Parent and Student are Living in Another Person’s Household

                        Two (2) Proofs of Residency [must have street address, no PO Box]

•     Post Office Form with forwarding address

•     Social Services check or unemployment check with new address

•      Pay Stub with new address

•      Driver’s License with new address

•    Notarized Letter from person you are living with stating your name, child’s name, address and relationship to that person

Parent Identification

• State Issued Driver’s License or State Issued Picture ID (one or both parents)
