The purpose of the screening program in schools is to identify students with potential impaiment that will require further follow up evaluation from a health care provider. Identifying students with possible impairment is necessary to provide the necessary assistance and accomodations to ensure the student is able to reach their full potential
NYS Mandates are as follows
Scoliosis- Girls Grade 5 and 7. Boys in grade 9
Vision and Hearing- K, 1, 3, 5, 7, 9, 11
Physical Exams are required for students in grades k, 1, 3, 5, 7, 9 and 11. If the above screenings are done by the students physician, the physicians findings will be noted and student will not be rescreened in school.

Information regarding requirements for sports participation
Athletes who wish to participate in sports teams at the Modified, Junior Varsity and Varsity levels are required to submit, prior to the beginning of each sports season:
- Registration is online at FamilyID.com Please complete online registration prior to start of sports. Call Health office with any questions or concerns.
- Physical Examination Form- Athletes must have a Physical exam by their provider every 12 months. Please call health office with any questions or concerns
- Medications- Students with asthma or allergies must follow specific requirements for providing medications prior to sports season.

Please note that physical exams are required for all new entrants and students entering grades K, 1,3,5,7,9, and 11. To be accepted for school entrance, physicals must be completed.
